## What is the LPC-662 form?

Illinois EPA’s source site certification for soil going to a clean construction or demolition debris (CCDD) fill operation or an uncontaminated soil fill operation. The source site owner or operator signs it to say the site is not potentially impacted property, the soil is presumed uncontaminated and its pH is between 6.25 and 9.0. Soil mixed with broken concrete, brick or asphalt needs it too.

The certification must be on Illinois EPA’s own form, so fill in this worksheet, then copy the answers into the LPC-662 PDF, print it, sign it and attach your evidence. ## LPC-662 vs LPC-663

LPC-662 is for soil from a site that is not potentially impacted property, and the owner or operator signs it. Soil from potentially impacted property (a past or current use, or contamination migrating from a nearby site, that makes contamination more likely) has to be sampled, tested against the maximum allowable concentrations and certified by a licensed professional engineer or geologist on LPC-663.

If an engineer or geologist decides the site is not potentially impacted, LPC-662 is still the form: the owner or operator signs and can attach the professional’s findings.

## How to fill out LPC-662

Section I locates the source site, with latitude and longitude to five decimal places. Section II names the site owner and the site operator. Section III describes current and past uses of the site and nearby property against the nine points the form lists, and counts the pages attached. Section IV gives the pH results and how the soil was sampled and tested.

Section V is signed by the owner, the operator, or an authorized representative of either. Where one load holds soil from several sites, the owner or operator of each site signs their own LPC-662.

## Questions

### Does every load need its own LPC-662?

Not usually. Illinois EPA says one certification per job site may be enough; agree it with the fill operation before the first load. Every load is still inspected on arrival, by eye and with a PID or FID meter, and any reading above background is rejected. ### Is pH testing required?

Yes, for all soil going to a CCDD or uncontaminated soil fill operation. No method is set: any reproducible, accurate method works, including SW-846 Method 9045D, ASTM D4972 or a field kit used as the maker says. Check the fill operation accepts your method. ### What if the soil is not going to a fill operation?
