## What is the LPC-663 form?

Illinois EPA’s certification that soil from potentially impacted property is uncontaminated soil, with a pH between 6.25 and 9.0, before it goes to a clean construction or demolition debris (CCDD) fill operation or an uncontaminated soil fill operation. A licensed professional engineer (PE) or licensed professional geologist (PG) evaluates the site, samples the soil, has it analysed and signs and seals the form.

## LPC 662 vs 663

LPC-662 is signed by the source site owner or operator, for soil from a site that is not potentially impacted property. LPC-663 is signed and sealed by a PE or PG, and is required for soil from potentially impacted property: a property where a past or current use, or contamination migrating from a nearby site, makes contamination more likely.

If the PE or PG concludes the site is not potentially impacted, the owner or operator signs LPC-662 instead and can attach the professional’s findings. Soil gathered from several places into one stockpile is certified on LPC-663 at the stockpile’s address.

## What goes with LPC-663

Section III references the attachments: where the soil was sampled and why those points are enough, and the lab results showing every constituent within the maximum allowable concentrations set under Part 1100, Subpart F, and the pH within 6.25 to 9.0. The lab package includes the chain of custody, the lab report, the lab’s accreditation status and its certification that the analysis meets the accreditation rules.

Analysis follows U.S. EPA SW-846. The sampling plan is the PE’s or PG’s decision, based on the site and the contaminants suspected.

## Questions

### Does a certified load still get checked at the fill site?

### Does each load need its own LPC-663?

### What happens to a rejected load?

The driver gets a written notice with the reasons. The load goes to a permitted landfill, or back to a fill operation only once the reasons are fixed and the soil is re-tested and certified again. ### Is the soil certification needed for painted concrete?
