1–4. Generator, operator, disposal site and agency
The waste generator fills in items 1 to 9 before the waste leaves.
For a demolition or renovation, the company and authorized agent doing the asbestos removal.
Write “on-site” if the waste is disposed of on the generator’s property.
The local, state or EPA Regional office that runs the asbestos NESHAP program where the work is.
What is an asbestos waste shipment record?
The record that goes with every load of asbestos-containing waste leaving a demolition, renovation or other NESHAP-covered job. The generator fills in the waste and where it is going, each transporter signs for it, and the landfill signs to say it arrived.
The federal rule asks for “a form similar to that shown in Figure 4”, so you can make your own as long as it holds the same items. Some states and landfills have their own version; use theirs where they require it.
How to fill out an asbestos waste shipment record
Items 1 to 4 name the work site and its owner, the contractor removing the asbestos, the disposal site and the agency that runs the asbestos NESHAP program there. Items 5 to 7 describe the waste: friable or not, the number and type of containers (DM, DP or BA) and the quantity in cubic yards. Item 8 holds anything special about handling or an alternate disposal site.
The generator’s authorized agent signs item 9 on the day the transporter takes the waste. Each transporter fills in and signs item 10 or 11. The disposal site notes any discrepancy in item 12 and signs item 13.
Who signs, and who keeps a copy
Every party keeps a copy. The disposal site sends a completed copy back to the operator in item 2 as soon as possible, and within 30 days of receiving the waste. Generators and disposal sites keep their records for at least 2 years.
If the generator has not had the signed copy back within 35 days of the transporter taking the waste, they must find out where it is. At 45 days they report in writing to the NESHAP office, with a copy of the record.

Do I have to use an official asbestos waste shipment record form?
Not under the federal rule. 40 CFR 61.150(d)(1) asks for the information on a form similar to EPA’s Figure 4, with no approved printer or numbered form. Check whether your state or the landfill asks for its own version. 40 CFR 61.150
Can it be signed on a phone?
The federal rule asks for a signed record but does not say how it is signed. Each person can sign their part on screen here; if the landfill or your agency wants ink, download the PDF, print it and sign in the boxes.
What if the signed copy never comes back from the landfill?
At 35 days after the transporter took the waste, contact the transporter or the disposal site. At 45 days, report in writing to the asbestos NESHAP office, with the record and a note of what you did to trace it. 40 CFR 61.150(d)
What does the landfill do if the waste doesn’t match the record?
It notes the difference in item 12. A significant amount of improperly enclosed or uncovered waste is reported to the agency by the following working day; a quantity difference not resolved within 15 days is reported to the agency straight away. 40 CFR 61.154(e)
How long do I keep it?
At least 2 years, for both the generator and the disposal site. 40 CFR 61.150(d)(5)
