1–5. Generator

What is the Illinois non hazardous special waste manifest?

The four-part form that goes with every load of non-hazardous special waste where the generator, the receiving facility or both are in Illinois. Non-hazardous special waste is industrial process and pollution control waste that is not RCRA hazardous; on construction jobs that is typically contaminated soil, sand and clay (codes IL01 and IL02). It is not for RCRA hazardous waste or TSCA waste, and construction or demolition debris is excluded from industrial process waste.

Illinois EPA prescribes the form but sells nothing: a third-party vendor can supply it, or the generator, transporter or facility can print it, as long as it is substantially identical. Fill in this worksheet, then print Illinois EPA’s blank and copy the answers across. The USEPA hazardous waste manifest or e-Manifest can be used instead.

How to fill out the special waste manifest

The generator fills in items 1 to 15. Item 4 is a number you make up: your IEPA ID number, the year and a sequence, such as 1234567890-2026-1. Items 6 and 7 need each transporter’s four-digit Illinois Special Waste Hauling Permit number, and item 8 the facility’s ten-digit Illinois ID number. Each waste gets a line in items 9 to 13, with its container type, quantity, unit and Illinois waste code.

The generator signs item 15 by hand; everything else may be pre-printed. Each transporter signs item 17 when it takes the load. The designated facility records any discrepancy or rejection in item 18, enters a management method code in item 19 and signs item 20 on receipt.

Illinois special waste codes

Item 13 takes one of 21 codes, IL01 to IL21. For construction: IL01 for leaking underground storage tank soil, sand and clay; IL02 for other contaminated soil, sand or clay; IL03 for other contaminated materials; IL13 for waste or used oil.

Questions

Who sends the manifest to Illinois EPA?

Nobody. No part of the manifest goes to Illinois EPA. The facility returns Part 2 to the generator, by post or scanned and emailed. Illinois EPA manifest instructions

Can the generator sign on a phone?

Not item 15: Illinois EPA’s instructions require a handwritten signature there. Print the form and sign it in ink. Someone signing for the generator can write “On behalf of” before signing. Illinois EPA manifest instructions

What if the facility rejects part of the load?

The facility notes it in item 18a. Partially rejected waste and residues travel on a new manifest that the rejecting facility starts and signs as offeror, with the original manifest number in item 14. Illinois EPA manifest instructions

How long do I keep the manifest?

Three years, for the generator, each transporter and the facility, available for Illinois EPA to inspect and copy. 35 Ill. Adm. Code 809.501
