What is the solid waste origin and disposal form?

The NJ O&D form goes with every load of solid waste a registered transporter takes to a New Jersey solid waste facility. It records who carried the load, in which registered equipment, what type of waste it was, the municipalities it came from and where it went, and the weight at the scale.

NJDEP provides the approved form and the transporter duplicates it for each load. The driver completes and signs part A before leaving for the disposal site and hands the form to the facility operator, who checks it, signs it and keeps it. A facility must turn away a load without a completed form signed by a registered transporter.

How to fill out the NJ O&D form

Part A is the transporter’s: the registered trade name and NJDEP registration number, the type of registration, whether the waste is self-generated, any lessor, the decal and plate for each piece of equipment, the waste type, where the waste came from by municipality, county and state with the share of the load from each, and the date collected. The driver prints their name, signs and dates item 10.

In part B the transporter names the final disposal facility and any non-hazardous manifest, bill of lading or pull ticket number. The weigh master enters the weigh location, gross weight, and for in-state disposal the net weight and scale ticket number, then signs item 16. For a load disposed of in New Jersey, the facility operator completes part C: permit or ID number, date, time and stamp or signature.

O&D form waste type codes

ID 10 municipal solid waste, ID 13 bulky waste, ID 13C construction and demolition waste, ID 23 vegetative waste, ID 25 animal and food processing waste, ID 27 dry industrial waste, ID 27A asbestos containing waste, ID 27I incinerator ash. Other types, such as ID 72 non-hazardous bulk liquids, go under Other.

ID 13C covers building material and rubble from construction, remodeling, repair and demolition: wood, concrete, asphalt, brick, block, plaster and wallboard, roofing, cardboard, metal, non-asbestos insulation, plastic, dirt, carpet and glass. Asbestos is ID 27A, not 13C. Waste processed at a transfer station keeps the type it arrived as.

Who does not need an O&D form

Anyone carrying only their own household waste in a car with general registration plates, and anyone carrying only their own solid waste in a vehicle registered at no more than 9,000 lb gross vehicle weight, or 16,000 lb combined with a trailer.

Facilities are also exempt from the O&D check when they are a non-commercial industrial facility taking only its own firm’s waste, a municipal facility taking only that municipality’s waste collected by its own transporters, or a recycling facility taking only recyclables separated where they were made.

Questions

Do I need a new O&D form for every load?

Yes. The transporter duplicates the approved form for each load of solid waste going to an in-State solid waste facility, and completes and signs it before the load leaves. N.J.A.C. 7:26-3.5(h)

Who signs the transporter’s certification?

The driver representing the transporter prints and signs their name and dates it, to certify that part A was completed accurately. O&D form instructions (PDF)

Can I fill in the O&D form on my phone?

Yes: fill in every item here, sign on screen and download the form as a PDF to hand over or print. Where the facility wants ink, print it and sign in the boxes.

What if the load goes out of state?

Fill in item 12 with the non-hazardous manifest or bill of lading number. The weigh master records the gross weight and certifies it in item 16. Part C is for loads disposed of in New Jersey only. O&D form instructions (PDF)

How long does the facility keep O&D forms?

At least 1 year, available for inspection by NJDEP, the county and the local health department during normal working hours. N.J.A.C. 7:26-2.13(c)
