What is a hazardous waste manifest?
EPA Form 8700-22, the Uniform Hazardous Waste Manifest, goes with every off-site shipment of hazardous waste from a small or large quantity generator. It names the generator, each transporter and the designated facility, describes each waste by its DOT shipping description and waste codes, and carries a signature at each change of hands.
Only two things count as a manifest: a paper form bought from an EPA-registered printer, with its tracking number already printed in item 4, or an electronic manifest made in EPA’s e-Manifest system. EPA’s sample form is for reference and cannot be used for a shipment.
How to fill out a hazardous waste manifest
The generator fills in items 1 to 15: the site’s EPA ID number, total pages, a 24-hour emergency number, the generator’s and site’s addresses, both transporters and the designated facility with their EPA ID numbers, and a line for each waste with its DOT description, containers, quantity, unit and up to six waste codes. Item 14 carries anything else the shipment needs.
The generator signs item 15 and gets the first transporter’s signature and date in item 17 before the waste leaves. Each transporter signs item 17 when they take it on, and the designated facility notes any discrepancy in item 18, enters a management method code for each waste in item 19 and signs item 20.
Hazardous waste manifest container type codes
Item 10 takes the number of containers and one code for their type: BA bags, CF fiber or plastic boxes, CM metal boxes including roll-offs, CW wooden boxes, CY cylinders, DF fiberboard or plastic drums, DM metal drums, DT dump truck, DW wooden drums, HG hopper or gondola cars, TC tank cars, TP portable tanks, TT cargo tanks.
Item 12 takes the unit: G gallons and L liters for liquids only, K kilograms, P pounds, and for very large bulk shipments only T tons of 2,000 pounds, M metric tons, N cubic meters and Y cubic yards.
Hazardous waste manifest retention requirements
The generator keeps the copy signed by the first transporter until the designated facility’s signed copy comes back, and keeps that for at least 3 years from the date the first transporter accepted the waste. The periods run on during any unresolved enforcement action.
If the facility’s signed copy has not come back, a large quantity generator contacts the transporter or facility at 45 days and files an exception report in e-Manifest at 60 days. A small quantity generator sends a copy marked as not confirmed to e-Manifest at 60 days.

Questions
Can I print a hazardous waste manifest myself?
No. Paper manifests come only from EPA-registered printers, each with its own pre-printed tracking number, in sets of four copies. The alternative is an electronic or hybrid manifest made in e-Manifest. EPA-registered manifest printers
Who pays for a manifest?
EPA charges the receiving facility for each manifest submitted, not the generator or transporter. For shipments from 1 October 2025 to 30 September 2027: $25 for a scanned paper manifest, $7 for data plus image, and $5 for a fully electronic or hybrid manifest. e-Manifest user fees
Does a very small quantity generator need a manifest?
Not under federal rules, as long as it meets the conditions in 40 CFR 262.14, including sending the waste only to a facility allowed to take it. States that run their own hazardous waste program can be stricter. 40 CFR 262.14
What if the signed copy never comes back?
A large quantity generator contacts the transporter or facility at 45 days and files an exception report in e-Manifest at 60 days. A small quantity generator sends a copy of the manifest, marked as not confirmed, to e-Manifest at 60 days. 40 CFR 262.42
How long do I keep a hazardous waste manifest?
