Each row comes from the document’s own rules, forms and guidance: the same sources the free tool for that document is built from. Who signs and what each person fills in follow the form part by part. Questions the tools do not need, such as whether a note can be signed on screen, were read from the regulator’s or the legislature’s own pages and are linked beside the answer.
